There are increasing numbers of centers using a right side laparoscopic radical nephrectomy with thrombectomy at last years but only few reports about laparoscopic management of IVC thrombus from left kidney.</p><p><bold>The study objective</bold>is to demonstrate reproducibility and relative safety of laparoscopic radical nephrectomy with thrombectomy of IVC tumor thrombus originating from the left kidney.</p><p><bold>Materials and methods</bold>. We describe the method and present the initial series of full laparoscopic level II-III IVC thrombectomy in three patients with tumor in left kidney. One patient had been diagnosed distant metastases before operation, the another — pancreas involvement in the tumor process. The follow up time consist 4—26 months after surgery.</p><p><bold>Results</bold>. All procedures completed without conversion to open surgery. Tumor sizes were 5—16 cm, length of thrombus in IVC — 2.4—7.0 cm. Volume of blood loss ranged from 300 to 2500 ml. One patient received blood transfusion after surgery. One patient have died because of distant metastases 5 months after surgery. Two others were alive at 4 and 26 months follow-up without signs of progression.</p><p><bold>Conclusion.</bold>Laparoscopic IVC tumor thrombectomy for level II-III thrombi in cases of left kidney cancer is feasible, safe. This kind of pro­cedure doesn’t lead to serious perioperative complications and can gain a quicker recovery after surgery. It needs more operations and longer follow-up for evaluation oncological efficiency.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold> Хирургическое лечение пациентов с опухолевым тромбом в нижней полой вене (НПВ) П—Ш уровней, исходящим из левой почечной вены, в большинстве случаев все еще остается большой открытой операцией, сопровождающейся осложнениями у 38 % па­циентов и периоперационной смертностью 4—10 %. Объем оперативного вмешательства подразумевает выполнение тромбэкто­мии из НПВ, радикальной нефрэктомии и ипсилатеральной лимфаденэктомии. В последние годы постепенно увеличивается число центров, внедряющих лапароскопические методики радикальных правосторонних нефрэктомий с тромбэктомией. Однако о вы­полнении подобных операций при тромбах в НПВ, исходящих их левой почки, можно встретить лишь единичные сообщения.</p><p><bold>Цель исследования</bold> — показать воспроизводимость и относительную безопасность методики лапароскопической радикальной нефрэктомии с тромбэктомией из НПВ опухолевого тромба, исходящего из левой почки.</p><p><bold>Материалы и методы</bold>. В настоящей статье представлены описание методики и собственный опыт выполнения лапароскопических радикальных нефрэктомий с тромбэктомией из НПВ у 3 пациентов с опухолевым тромбом П—Ш уровней, исходящим из левой почки. У1 пациента на момент операции были выявлены отдаленные метастазы, у другого — прорастание опухоли в хвост поджелудочной железы. Период наблюдения после операции составляет 4—26мес.</p><p><bold>Результаты</bold>. Конверсий к открытой хирургии не было. Максимальный размер опухоли составлял от 5 до 16 см. Протяженность опухолевого тромба в НПВ составляла 2,4—7,0 см. Объем кровопотери — 300—2500мл. Одному пациенту в послеоперационном периоде потребовались гемотрансфузии. Через 5мес 1 пациент умер от прогрессирования заболевания, остальные живы без при­знаков рецидива.</p><p><bold>Заключение</bold>. Представленные наблюдения позволяют говорить о воспроизводимости и относительной безопасности предлагаемой методики, позволяющей соблюсти принципы онкологии и сосудистой хирургии. Операция не приводит к серьезным периоперационным осложнениям и сопровождается довольно благоприятным течением раннего послеоперационного периода.
