Patients at its high risk are characterized by a more aggressive course of the disease and significantly lower tumor-specific and relapse-free survival rates. Hormone therapy and radiotherapy are one of the conventional treatments in patients with PC at high risk of progression. Nonetheless, more and more publications demonstrating the efficiency and safety of surgical therapy in this contingent of patients are recently appearing. This paper presents the results of surgical treat-ment in 499 patients with PC at high risk of progression, who have undergone radical prostatectomy with extended pelvic lymphadenectomy at the Department of Urologic Oncology, P.A. Herzen Moscow Oncology Research Institute.</p><p> </p></abstract><trans-abstract xml:lang="ru"><p>Рак предстательной железы (РПЖ) является актуальной проблемой современной онкоурологии. Больные подгруппы высокого риска характеризуются более агрессивным течением заболевания и достоверно меньшими показателями опухолевоспецифической и безрецидивной выживаемости. Одним из общепринятых методов терапии больных РПЖ с высоким риском прогрессирования является гормонолучевое лечение. Тем не менее в последнее время появляется все больше работ, демонстрирующих эффективность и безопасность хирургического метода терапии данного контингента больных. В статье представлены результаты хирургиче-ского лечения 499 больных РПЖ с высоким риском прогрессирования, которым выполнена радикальная простатэктомия с расши-ренной тазовой лимфаденэктомией в отделении онкоурологии МНИОИ им. П.А.
