Patients were treated with avelumab (800 mg every two weeks) and axitinib (5 mg twice daily). Primary endpoints were median progressionfree survival (PFS) and objective response rate (ORR). Secondary endpoints included median overall survival, 1-year overall survival, and safety. Subgroup analyses evaluated median PFS and ORR in patients with ³2 organ metastases, bone metastases, and liver metastases.</p><p><bold>Results.</bold><bold> </bold>A total of 125 patients from 13 centers were enrolled, with median follow-up of 16.1 months and median age of 61 years. Based on IMDC (International Metastatic RCC Database Consortium) risk categories, 35.3 % had favorable, 49 % had intermediate, and 15.7 % had poor prognosis. The overall population demonstrated median PFS of 14.9 months and ORR of 44.3 %. Among patients with ³2 organ metastases, median PFS was 13.0 months, and ORR was 36.7 % (all <italic>p </italic>&gt;0.05). In those with bone metastases, median PFS was 6.5 months (<italic>p</italic><italic> </italic>= 0.160), and ORR was 23.5 % (<italic>p</italic><italic> </italic>= 0.0148). For patients with liver metastases, median PFS was 17.6 months, and ORR was 45.5 % (all <italic>p </italic>&gt;0.05).</p><p><bold>Conclusion. </bold>The combination of avelumab and axitinib demonstrated consistent efficacy in patients with advanced disease including those with liver and bone metastases. These findings suggest the regimen’s broad applicability and suitability in real-world clinical practice.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение. </bold>В проспективных рандомизированных исследованиях иммунотаргетная терапия продемонстрировала впечатляющие результаты и стала стандартом лечения пациентов с метастатическим почечно-клеточным раком.</p><p><bold>Цель </bold>настоящего <bold>анализа </bold>в рамках исследования реальной практики RAVE-Renal – оценка эффективности комбинации авелумаба и акситиниба в зависимости от степени распространения заболевания и наличия метастазов в печени и костях.</p><p><bold>Материалы</bold><bold> </bold><bold>и методы. </bold>RAVE-Renal – многоцентровое амбиспективное исследование, в которое включены нелеченые пациенты с гистологически подтвержденным метастатическим почечно-клеточным раком и измеряемыми очагами. Пациенты получали авелумаб (800 мг каждые 2 нед) и акситиниб (5 мг 2 раза в день). Первичными конечными точками были медиана выживаемости без прогрессирования и частота объективных ответов. Вторичными конечными точками – медиана общей выживаемости, 1-летняя общая выживаемость и безопасность. Подгрупповой анализ оценивал медиану выживаемости без прогрессирования и частоту объективных ответов в группах пациентов с метастазами в ³2 органах, метастазами в костях и печени.</p><p><bold>Резу</bold><bold>льтаты.</bold><bold> </bold>Всего были включены 125 пациентов из 13 центров, медиана наблюдения составила 16,1 мес. Медиана возраста пациентов – 61 год. Исследуемая популяция включала 35,3 % пациентов с благоприятным, 49 % с промежуточным и 15,7 % с неблагоприятным прогнозом по IMDC (International Metastatic RCC Database Consortium, Международный консорциум по лечению метастатического рака почки). В общей популяции медиана выживаемости без прогрессирования составила 14,9 мес, частота объективных ответов – 44,3 %.
