However, poor-risk patients represented only a small proportion of the trial population.</p><p><bold>Aim. </bold>This multicenter retrospective cohort study assessed the real-world efficacy and safety of lenvatinib plus pembrolizumab in patients with clear-cell metastatic RCC and intermediate or poor International Metastatic RCC Database Consortium risk.</p><p><bold>Materials and methods. </bold>Outcomes included objective response rate, progression-free survival, overall survival, and safety. Sixty patients were analyzed, with a median age of 56 years. Poor risk was identified in 53 % of patients, and 90 % had metastases to ≥2 organs.</p><p><bold>Results. </bold>Objective response rate was 48.33 %, disease control rate was 86.7 %, and median progression-free survival was 19.0 months. Grade ≥3 adverse events occurred in 25 % of patients, with 33.3 % requiring lenvatinib dose reductions.</p><p><bold>Conclusion.</bold><bold> </bold>Lenvatinib plus pembrolizumab demonstrated robust efficacy and a manageable safety profile in a real-world population with advanced disease and poor-risk features, consistent with outcomes reported in clinical trials.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold><bold> </bold>Комбинация ленватиниба с пембролизумабом продемонстрировала высокую эффективность у пациентов с метастатическим почечно-клеточным раком в исследовании III фазы CLEAR, однако пациенты группы высокого риска составляли лишь небольшую часть исследуемой популяции.</p><p><bold>Цель исследования </bold>– в условиях реальной клинической практики оценить эффективность и безопасность комбинации ленватиниба с пембролизумабом у больных светлоклеточным метастатическим почечно-клеточным раком промежуточного и высокого риска согласно критериям Международного консорциума по лечению метастатического рака почки (International Metastatic RCC Database Consortium, IMDC).</p><p><bold>Материалы</bold><bold> </bold><bold>и методы. </bold>В многоцентровом ретроспективном исследовании проанализированы данные 60 пациентов с медианой возраста 56 лет. К группе высокого риска отнесены 53 % пациентов, у 90 % больных были метастазы более чем в 2 органах. В качестве конечных точек были выбраны частота объективного ответа, выживаемость без прогрессирования, общая выживаемость и безопасность.</p><p><bold>Р</bold><bold>езу</bold><bold>ль</bold><bold>таты.</bold><bold> </bold>Частота объективного ответа составила 48,33 %, частота контроля над заболеванием – 86,7 %, медиана выживаемости без прогрессирования – 19,0 мес.
