In 2022, 48,025 new PCa cases and 12,896 deaths from the disease were reported in Russia. Metastatic castration-resistant PCa still is one of the common causes of death in men.</p><p><bold>Aim.</bold><bold> </bold>To evaluate prevalence of homologous recombination repair gene (HRR) mutations in patients with metastatic castration-resistant PCa in Russia and to identify differences in clinical characteristics and treatment outcomes of patients with these mutations and without them.</p><p><bold>Materials</bold><bold> </bold><bold>and</bold><bold> </bold><bold>methods.</bold><bold> </bold>The study included 329 patients with metastatic castration-resistant PCa from 20 centers. Patients underwent molecular genetic analysis using next-generation sequencing to identify mutations in 14 HRR genes. The mutation status was determined in 3 routine laboratories and further validated in a central independent laboratory.</p><p><bold>Results.</bold><bold> </bold>The HRR gene mutations were detected in 59 (19.28 %) of 329 patients. The most common were <italic>ATM</italic><italic> </italic>mutations – in 14 (4.57 %), <italic>BRCA2</italic><italic> </italic>– in 14 (4.57 %), <italic>BRCA1 </italic>– in 6 (1.96 %). All the patients received first-line therapy, with newgeneration antiandrogens and taxanes being used most frequently. The patients with HRR gene mutations had nearly</p><p>1.5 times lower median of progression-free survival (PFS) with first-line therapy: 12.8 months <italic>vs</italic><italic>.</italic><italic> </italic>20.8 months in patients without mutations (<italic>p </italic>= 0.048). There was statistically significant difference in PFS between the observed groups with first-line therapy depending on the treatment group. In patients of general group having been treated with new-generation antiandrogen therapy, the median of PFS was 22 months, while in patients who had received taxanes it was 8.22 months (<italic>p </italic>&lt;0.05). In patients with HRR gene mutations having been treated with first-line antiandrogenes of novel generation, the median of PFS was 20.5 months, while in patients without mutations the PFS median was 23.1 months (<italic>p </italic>= 0.14). In patients with mutations and having been treated with first-line taxanes, the PFS median was 6.15 months, while in patients without mutations it was 8.55 months (<italic>p </italic>= 0.4).</p><p><bold>Conclusion.</bold><bold> </bold>In our ADAM study in the Russian population, the proportion of carriers of the HRR gene mutations was slightly lower (19.28 %) than that reported in published randomized trials. There is also a different distribution structure and frequency of the most frequent mutations. Patients with the HRR gene mutations exhibited worse PFS rates and more aggressive course of the disease, which requires a special approach to treatment of this group of patients.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение. </bold>Рак предстательной железы (РПЖ) – одно из самых распространенных онкологических заболеваний мужчин. В 2022 г. в России было зарегистрировано 48 025 новых случаев РПЖ и 12 896 случаев смерти от этого заболевания. Метастатический кастрационно-резистентный РПЖ продолжает оставаться одной из распространенных причин смерти мужчин.</p><p><bold>Цель</bold><bold> </bold><bold>исследования</bold><bold> </bold>– определить распространенность мутаций генов гомологичной рекомбинации (HRR) у больных метастатическим кастрационно-резистентным РПЖ в России и выявить различия в клинических характеристиках и исходах лечения пациентов с данными мутациями и без них.</p><p><bold>Ма</bold><bold>териалы</bold><bold> </bold><bold>и методы. </bold>В исследование включены 329 больных метастатическим кастрационно-резистентным РПЖ из 20 центров. Пациентам проводили молекулярно-генетический анализ с использованием секвенирования нового поколения для выявления мутаций в 14 генах HRR. Статус мутаций определяли в 3 рутинных лабораториях и дополнительно валидировали в центральной независимой лаборатории.</p><p><bold>Резу</bold><bold>льта</bold><bold>ты. </bold>Мутации генов HRR выявили у 59 (19,28 %) из 329 пациентов. Наиболее часто встречались мутации <italic>А</italic><italic>ТМ</italic><italic> </italic>– у 14 (4,57 %), <italic>BRCA</italic><italic>2</italic><italic> </italic>– у 14 (4,57 %), <italic>BRCA</italic><italic>1</italic><italic> </italic>– у 6 (1,96 %). Терапию 1-й линии получили все пациенты, при этом антиандрогены нового поколения и таксаны использовали чаще всего. У пациентов с мутациями генов HRR медиана выживаемости без прогрессирования (ВБП) при терапии 1-й линии была почти в 1,5 раза меньше: 12,8 мес против 20,8 мес у пациентов без мутаций (<italic>p</italic><italic> </italic>= 0,048). Выявлено статистически значимое различие между исследуемыми группами по ВБП при терапии 1-й линии в зависимости от группы препаратов. У пациентов общей группы медиана ВБП при терапии антиандрогенами нового поколения составила 22 мес, в то время как у пациентов, получивших таксаны, – 8,22 мес (<italic>p</italic><italic> </italic>&lt;0,05). У пациентов с мутациями генов HRR, получавших терапию 1-й линии антиандрогенами нового поколения, медиана ВБП составила 20,5 мес, в то время как у пациентов без мутаций – 23,1 мес (<italic>p</italic><italic> </italic>= 0,14). У пациентов с мутациями, получавших терапию 1-й линии таксанами, медиана ВБП составила 6,15 мес, в то время как у пациентов без мутаций – 8,55 мес (<italic>p</italic><italic> </italic>= 0,4).</p><p><bold>Заключение</bold><bold>.</bold><bold> </bold>В исследовании ADAM в российской популяции доля носителей мутаций генов HRR оказалась чуть ниже (19,28 %), чем в опубликованных рандомизированных исследованиях. Также отмечаются иные структура распределения и частота наиболее распространенных мутаций.
