Patients with ECOG (Eastern Cooperative Oncology Group) score 0–3, any Karnofsky scale status, and no detection of PD-L1 expression in the tumor were included. Follow-up period was 24 months.</p><p><bold>Results. </bold>Objective response rate was 68 % which is consistent with the CLEAR trial value of 72 %. Six-month PFS was 82 %, 12-month PFS was 70 %; 6-month OS was 91.2 %, 12-month OS was 83.4 %. Median PFS and OS were not reached. There were no new safety signals for the lenvatinib and pembrolizumab combination. Grade III adverse events were arterial hypertension and hepatotoxicity.</p><p><bold>Conclusion.</bold><bold> </bold>In real clinical practice, lenvatinib and pembrolizumab combination showed results consistent with the data of the basic clinical trial despite short follow-up and expanded inclusion criteria.</p></abstract><trans-abstract xml:lang="ru"><p><bold>В</bold><bold>ведение.</bold><bold> </bold>Комбинация ленватиниба с пембролизумабом продемонстрировала статистически значимые и клинически важные улучшения показателей выживаемости без прогрессирования (ВБП), общей выживаемости (ОВ) и частоты объективного ответа по сравнению с сунитинибом.</p><p><bold>Цель исследования </bold>– оценить эффективность и безопасность комбинации ленватиниба с пембролизумабом у пациентов с метастатическим почечно-клеточным раком в реальной клинической практике в Республиканском клиническом онкологическом диспансере (Уфа).</p><p><bold>Материалы</bold><bold> </bold><bold>и методы. </bold>В исследование были включены 24 пациента (20 мужчин и 4 женщины) со светлоклеточным почечно-клеточным раком. Включались пациенты со статусом по шкале ECOG (Eastern Cooperative Oncology Group, Восточная кооперативная группа исследования рака) 0–3 балла, любым статусом по шкале Карновского, без определения экспрессии PD-L1 в опухоли. Срок наблюдения – 24 мес.</p><p><bold>Резуль</bold><bold>та</bold><bold>ты.</bold><bold> </bold>Частота объективного ответа составила 68 %, что сопоставимо с показателем в исследовании CLEAR, равным 72 %. Показатель 6-месячной ВБП составил 82 %, 12-месячной ВБП – 70 %; 6-месячной ОВ – 91,2 %, 12-месячной ОВ – 83,4 %. Медиана ВБП и ОВ не были достигнуты. Новых сведений о безопасности комбинации ленватиниба с пембролизумабом не получено.
