Unfortunately, the procedure is associated with very high complication rate exceeding 50 %.</p><p><bold>Aim. </bold>To perform retrospective comparative analysis of oncological outcomes, intraoperative and early postoperative complications of open and endoscopic inguinofemoral lymphadenectomy in treatment of patients with penile cancer.</p><p><bold>Materials and methods. </bold>Treatment outcomes of 64 patients with penile cancer who underwent open or video endoscopic lymphadenectomy were analyzed. Primary and secondary surgical treatment outcomes were evaluated.</p><p><bold>Results. </bold>Open inguinofemoral lymphadenectomy was performed in 54 patients, video endoscopic in 10. Analysis of primary surgical results showed a significant (3-fold) reduction of the duration of lymphorrhea and the duration of hospitalization (2-fold) in patients of the video endoscopic lymphadenectomy group. There was no difference in the number of removed lymph nodes in the two types of surgeries. Average operative time for video endoscopic lymphadenectomy was 15–20 % longer than operative time of open intervention. Analysis of safety indicators showed that during open surgery, the frequency of wound infection was 24 %, skin flap necrosis – 55.5 %, wound dehiscence – 52 %, lymphedema – 15 %. No such complications were observed in lateral video endoscopic inguinofemoral lymphadenectomy.</p><p><bold>Conclusion. </bold>Video endoscopic inguinofemoral lymphadenectomy has significant advantages compared to open method in terms of reduction of the rate of surgical complications and duration of hospitalization.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение. </bold>В лечении инвазивного и местно-распространенного рака полового члена диссекция паховых лимфатических узлов является важным этапом. К сожалению, процедура сопровождается осложнениями, частота которых превышает 50 %.</p><p><bold>Цель исследования</bold> – ретроспективный сравнительный анализ онкологических результатов, интраоперационных и ранних осложнений открытой и эндоскопической пахово-бедренной лимфаденэктомии при раке полового члена.</p><p><bold>Материалы и методы.</bold> Проанализированы результаты лечения 64 пациентов с раком полового члена, подвергнутых открытой или видеоэндоскопической лимфаденэктомии. Оценивали первичные и вторичные хирургические результаты лечения.</p><p><bold>Результаты.</bold> Открытая пахово-бедренная лимфаденэктомия выполнена 54 пациентам, видеоэндоскопическая – 10. При анализе первичных хирургических результатов выявлено существенное сокращение длительности лимфореи (в 3 раза) и сроков госпитализации (в 2 раза) у пациентов группы видеоэндоскопической лимфаденэктомии. Не выявлено разницы в количестве удаленных лимфатических узлов в зависимости от модификации операции. Среднее время видеоэндоскопической лимфаденэктомии на 15–20 % превышает длительность открытого вмешательства. При анализе показателей безопасности отмечено, что при открытой операции частота раневой инфекции составила 24 %, некроза кожных лоскутов – 55,5 %, расхождения краев ран – 52 %, лимфедемы – 15 %. При латеральной видеоэндоскопической ингвинальной лимфаденэктомии подобных осложнений не наблюдалось.</p><p><bold>Заключение</bold>.
