This study included 122 patients with histologically verified stage Т1–2N0M0 PCa treated in N.A. Lopatkin Research Institute of Urology and Interventional Radiology, a branch of the National Medical Research Radiology Center. Their mean age was 65.2 ± 6.8 years. All patients underwent multiparametric magnetic resonance imaging and fusion biopsy (samples were collected from targeted areas in combination with a standard biopsy), followed by histological examination of biopsy samples and mapping of the affected areas. Twenty-eight patient underwent RPE.</p><p><bold>Results</bold>. Study participants were distributed as follows by their Prostate Imaging Reporting and Data System (PI-RADS) score: 5 points in 23 patients (18.9 %), 4 points in 57 patients (46.7 %), and 3 points in 42 patients (34.4 %). Targeted biopsy revealed cancer in 105 out of 122 patients (86.1 %) in at least one sample. Seventeen targeted biopsy samples were negative. The majority of patients had ISUP (International Society of Urological Pathology) grade 1 (n = 57; 46.8 %) and grade 2 (n = 33; 27.0 %) tumors. Comparison of biopsy findings and results of pathological examination of the removed prostate demonstrated significant discrepancies in the distribution of cases by their ISUP grades. Almost half of the patients (46.8 %) were diagnosed with ISUP grade 1 tumors after biopsy, while the assessment of RPE specimen demonstrated ISUP grade 1 tumors only in 21.4 % of patients. ISUP grade 3 tumors were diagnosed in 16.4 % and 35.7 % of patients using biopsy- and RPE-derived specimens, respectively. The proportions of ISUP grade 2, 4, and 5 tumors were similar with both methods. In 27 out of 28 cases, examination of biopsy- and RPE-derived specimens demonstrated complete agreement in terms of tumor location. Importantly, this study included only patients with histologically confirmed PCa; so we didn’t evaluate the effectiveness of biopsy for cancer diagnosis. Focal therapy could have been considered in 22 out of 28 patients (78.6 %) who underwent RPE. This proportion of focal therapy-eligible patients is almost equal to that identified by fusion biopsy (75.4 %).</p><p><bold>Conclusion.</bold> Multiparametric magnetic resonance imaging in combination with targeted and multifocal prostate biopsy allow precise identification of candidates eligible for focal therapy with a relatively low risk of an erroneous conclusion.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold> Методы фокальной терапии рака предстательной железы (РПЖ) позволяют сохранить качество жизни, но ограничением к использованию этой стратегии является отсутствие уверенности в определении точной локализации опухолевых очагов.</p><p><bold>Цель исследования</bold> – оценка возможности отбора пациентов на фокальные методы терапии с помощью fusion-биопсии с последующим подтверждением результатами радикальной простатэктомии.</p><p><bold>Материалы и методы</bold>. В анализ вошли данные обследования 122 пациентов, получавших лечение в НИИ урологии и интервенционной радиологии им. Н.А. Лопаткина – филиале НМИЦ радиологии, с гистологически верифицированным РПЖ Т1–2N0M0. Средний возраст составил 65,2 ± 6,8 года. Всем пациентам выполняли мультипараметрическую магнитно-резонансную томографию и fusion-биопсию (биоптаты забирали из таргетных зон в комбинации со стандартной биопсией) с последующим гистологическим исследованием биоптатов и картированием очагов в предстательной железе. Радикальная простатэктомия в дальнейшем была выполнена 28 пациентам.</p><p><bold>Результаты.</bold> Распределение больных в соответствии с критериями PI-RADS (Prostate Imaging Reporting and Data System): 5 баллов – у 23 (18,9 %) больных, 4 балла – у 57 (46,7 %), 3 балла – у 42 (34,4 %). При выполнении таргетной биопсии рак выявлен в 105 (86,1 %) из 122 случаев по крайней мере в 1 из биоптатов. В 17 наблюдениях в таргетных биоптатах рак не обнаружен. При биопсии у большинства больных по классификации ISUP (Международного общества урологических патологов) выявляли опухоль градации ISUP 1 (у 57 (46,8 %) пациентов) и ISUP 2 (у 33 (27,0 %) пациентов). При сравнении результатов биопсии и патологоанатомического исследования удаленной железы обнаружено, что распределение случаев по принадлежности к группам по градации ISUP существенно различалось. Градация опухоли ISUP 1 при биопсии и радикальной простатэктомии выявлена в 46,8 и 21,4 %, ISUP 3 – в 16,4 и 35,7 % случаев соответственно. Процентное содержание случаев выявления РПЖ ISUP 2, 4 и 5 было сопоставимо. При этом отмечается полное совпадение данных о локализации опухоли по результатам биопсии и радикальной простатэктомии в 27 из 28 случаев. Следует учитывать, что в исследование были включены только пациенты с гистологическии подтвержденным РПЖ и мы не оценивали эффективность биопсии в выявлении рака. Вопрос о проведении фокальной терапии можно было бы обсуждать в отношении 22 (78,6 %) из 28 пациентов, которым выполнили радикальную простатэктомию, что практически совпадает с оценкой возможности выполнения фокальной терапии на основании fusion-биопсии – 75,4 %.</p><p><bold>Заключение.</bold> Современные возможности использования мультипараметрической магнитно-резонансной томографии в сочетании с таргетной и полифокальной биопсией предстательной железы позволяют с высокой степенью надежности отбирать кандидатов с РПЖ на фокальную терапию с относительно небольшим риском ошибочного заключения.</p></trans-abstract><kwd-group xml:lang="en"><kwd>fusion biopsy</kwd><kwd>prostate</kwd><kwd>focal therapy</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>fusion-биопсия</kwd><kwd>предстательная железа</kwd><kwd>фокальная терапия</kwd></kwd-group><funding-group><funding-statement xml:lang="en">The study was performed without external funding</funding-statement><funding-statement xml:lang="ru">Исследование проведено без спонсорской поддержки</funding-statement></funding-group></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Petrova G.V., Starinskiy V.V., Gretsova O.P. et al.
