Prostate cancer (PCa) patients often develop recurrent disease after radical surgery. A tool that can accurately predict the risk of disease progression in the population of Russian patients will be very helpful to choose an optimal treatment strategy and prevent possible recurrence.</p><p><bold>Objective</bold>: to analyze preoperative and postoperative prognostic factors for PCa progression and identify the most significant of them.</p><p><bold>Materials and methods</bold>. This study included 2,255 patients with localized and locally advanced PCa who underwent radical surgery. We constructed nomograms for predicting the risk of disease progression after surgery using mathematical models.</p><p><bold>Results</bold>. We created nomograms for predicting the risk of biochemical recurrence and probability of relapse-free survival by the level of prostate specific antigen (PSA) in patients with no lymph node metastases (pN0) according to the results of morphological examination and in patients with lymph node metastases (pN1). The accuracy of nomograms reached 71 % (area under the ROC curve (AUC) 0.7119) and 76 % (AUC 0.7617), respectively.</p><p><bold>Conclusion</bold>. The nomograms demonstrated high accuracy of prognosis and can be used in the population of Russian patients.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение</bold>. Рак предстательной железы - заболевание, после радикального хирургического лечения которого нередко возникают рецидивы. Для подбора дальнейшей тактики лечения и предотвращения возможного рецидива необходим инструмент, который с высокой точностью поможет спрогнозировать риск прогрессирования заболевания у российской популяции пациентов.</p><p><bold>Цель исследования</bold> - провести статистический анализ пред- и послеоперационных факторов прогноза, влияющих на прогрессирование рака предстательной железы, отобрать наиболее значимые факторы.</p><p><bold>Материалы и методы</bold>. В исследование были включены 2255 пациентов с клинически локализованным и местно-распространенным раком предстательной железы, которым выполнено радикальное хирургическое лечение. С помощью математических моделей рассчитаны и построены номограммы, прогнозирующие риск прогрессирования заболевания после хирургического лечения.</p><p><bold>Результаты</bold>. Созданы номограммы для прогнозирования вероятности развития биохимического рецидива и безрецидивной выживаемости по уровню простатического специфического антигена у больных с отсутствием метастазов в лимфатических узлах (pN0) по результатам планового морфологического исследования и у пациентов с наличием метастазов в лимфатических узлах (pN1) с точностью прогноза 71 % (площадь под ROC-кривой (AUC) 0,7119) и 76 % (AUC 0,7617) соответственно.</p><p><bold>Заключение</bold>.
