The incidence and mortality of PCa is constantly rising in Russia. The rate of annual increase in the number of new PCa cases is the highest among all malignancies in Russia and reaches 87.7 % over 10 years (from 2008 to 2018). Approximately one-third of patients have high-risk PCa. This article analyzes the outcomes of surgical treatment in high-risk PCa patients.</p><p><bold>Objective</bold>: to evaluate survival of patients with high-risk PCa after surgical treatment and analyze prognostic factors affecting survival.</p><p><bold>Materials and methods.</bold> This retrospective study included 1507 high-risk PCa patients who have undergone surgical treatment at P.A. Herzen Moscow Oncology Research Institute. Study participants were divided into groups to assess the effect of preoperative prognostic factors on patient survival after surgery. The following factors were analyzed: level of prostate specific antigen &gt;20 ng/mL, Gleason score 8—10, and сТ3—4 clinical stage.</p><p><bold>Results</bold>. Median follow-up time was 58 months. The five-year biochemical relapse-free survival rate in the general group was 57.4 %; five-year overall and cancer-specific survival rates were 94.4 % and 97.7 %, respectively. The incidence of biochemical relapses and survival rates depended on the presence and number of risk factors in the groups studied (p &lt;0.0001). The frequency of local relapses and disease progression also depended on the number of risk factors: the more poor prognostic factors the patient had, the lower was the survival.</p><p><bold>Conclusion.</bold> To find an optimal treatment strategy for high-risk PCa patients after surgery, we recommend using an extended classification (depending on the number of risk factors), since the number of prognostic factors affects survival rates.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение. </bold>Рак предстательной железы (РПЖ) на сегодняшний день остается одним из наиболее актуальных вопросов онкологии. В России ежегодно увеличиваются заболеваемость и смертность от РПЖ. Темп ежегодного прироста числа заболевших РПЖ мужчин является самым высоким среди всех злокачественных новообразований в России и составляет 87,7 % за 10 лет (с 2008 по 2018 г.). Около трети больных с впервые выявленным РПЖ относятся к группе высокого риска прогрессирования. В данной статье приведены результаты хирургического лечения больных РПЖ высокого риска прогрессирования.</p><p><bold>Цель исследования</bold> — оценка показателей выживаемости больных РПЖ высокого риска прогрессирования после хирургического лечения. Анализ факторов прогноза, влияющих на показатели выживаемости больных данной категории.</p><p><bold>Материалы и методы</bold>. Проведен ретроспективный анализ базы данных, включающей 1507 больных РПЖ высокого риска прогрессирования, которым выполняли хирургическое лечение в МНИОИ им. П.А. Герцена. Для определения степени влияния предоперационных факторов прогноза на показатели выживаемости после хирургического лечения больных распределили по группам (наличие одного или нескольких факторов: уровень простатического специфического антигена &gt;20 нг/мл, дифференцировка опухоли по шкале Глисона 8—10 и клиническая стадия сТ3—4).</p><p><bold>Результаты</bold>. Медиана периода наблюдения составила 58 мес. Показатели 5-летней биохимической безрецидивной выживаемости в общей группе больных составили 57,4 %, 5-летней общей и опухолевоспецифической выживаемости — 94,4 и 97,7 % соответственно. В зависимости от наличия и количества неблагоприятных факторов прогноза в группах больных РПЖ биохимический рецидив регистрировали с различной частотой, также выявлены различия показателей выживаемости (р &lt;0,0001). В зависимости от числа неблагоприятных факторов прогноза в группах больных РПЖ частота выявления местного рецидива и прогрессирования также различалась: по мере увеличения количества неблагоприятных факторов прогноза показатели выживаемости снижались.</p><p><bold>Заключение.</bold> Для определения рациональной лечебной тактики после хирургического лечения больных РПЖ высокого риска рекомендовано использовать расширенную классификацию (в зависимости от количества неблагоприятных факторов), поскольку количество факторов прогноза оказывает влияние на показатели выживаемости.</p></trans-abstract><kwd-group xml:lang="en"><kwd>prostate cancer</kwd><kwd>high risk</kwd><kwd>surgical treatment</kwd><kwd>cancer outcomes</kwd><kwd>prognostic factor</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>рак предстательной железы</kwd><kwd>высокий риск</kwd><kwd>хирургическое лечение</kwd><kwd>онкологические результаты</kwd><kwd>фактор прогноза</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Pernar C., Ebot E., Wilson K. et al.
